If the harrowing challenges aren’t enough, you’ll also have the chance to earn shiny, golden trophies for those elite few Directors who can actually survive. In Challenge Mode, conditions start to mutate into even deadlier strains, making it a race against time to research cures before your hospital becomes overwhelmed. Mutated conditions come with much higher mortality rates and infectiousness, so it’s up to you to treat as many patients as possible, put your research nerds to work, and rely on consultants to help manage the chaos. Keep a close eye on these new threats or you will end up with a financial Nightmare of patient fatalities before you can find a cure.

You’ll also be dealing with a massive influx of patients. Infection rates are through the roof, and thanks to HEAL turning off its “KPI safeties” (because apparently, they were illegal), patients will be flooding in non-stop. They’ll be more demanding, needing rooms you haven’t built yet or clogging up queues for rooms that are already full. Sandbox events will make things even crazier with waves of patients swarming your hospital.

And yes there will be more death. A lot more. Patients in Challenge Mode show up with significantly lower health, meaning you’ll need to act fast to prevent them from dying on your watch. Their health deteriorates quicker, and treatment becomes tougher, so you’ll need top-tier doctors to keep them alive. Your disposal tech might cost a fortune, but with the state of these patients, you’re about to save a ton on body bags.

As if things weren’t bad enough, medical debt has crushed the economy, so now both you and your patients are broke. Income from treatments has been slashed, and that travelling merchant who used to hand out freebies? Not anymore. You’ll have to train your doctors up from scratch, as Level 3 recruits are off the table.

Every part of the game has been cranked up to push you harder. From more frequent disasters to patients that are even less patient, you’ll need to pull out all the stops to keep everything under control. There’s even a new loss condition—if too many patients die, the “Space Cops” will swoop in and shut down your hospital.
